Travel Vaccination

Please note that we request 8 weeks’ notice for submission of the travel vac application form (due to it taking this long for completion of some vaccination schedules).

We are a Yellow Fever Vaccination Centre and can offer travel health advice and appropriate vaccinations for most destinations in the world.

To access this, download a pdf Travel Vaccination Form from Patients Documents section of this website or call in at reception who can give you one. Use the form to set out your travel itinerary. Use a separate form for each person travelling as their requirements may be different.

Please hand the form in at Reception or post to us at:
Bourne Galletly Practice, 40 North Road, Bourne, Lincolnshire, PE10 9BT.

Our trained nurses will then work out what vaccinations are required for each person. Our receptionist will then contact you with the cost of the vaccinations and once payment has been received you will be booked in for the vaccinations.
