This short online form is the quickest and simplest way for the majority of our patients to access or clinical team for assistance. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.

Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 1800 – 0900. Please contact 111 during out of hours or 999 if this is an emergency.

We operate a telephone callback system, this ensures that all patients receive appropriate medical intervention by a GP on the same day that they make a request, this system speeds up your care.

How this works

Our reception staff will make a telephone call back appointment with a GP. We aim that this will be within 1 hour of your initial call unless you ask for an alternative time. If you are at work and wish to have a call during a break then please advise our reception staff when you will be available.

Our receptionists will ask you a few simple questions:

1) They will confirm your details

2) Ask you if you wish to see a preferred GP

3) Ask you for brief details of your problem.

The duty Doctor or Triage Nurse will call you back, discuss your symptoms and plan the most appropriate course of action.

The information requested will help our doctors prepare prior to speaking with you. While you do not have to give this information, we encourage our patients to do so as it is of great assistance to the medical team in preparing for your consultation. We understand that some patients do not wish to provide such details to non-clinical staff and we respect this view. For your confidence however, we should advise that all our surgery staff are bound by patient confidentiality and have received training in Information Governance and the data Protection Act. We will do all that we can to ensure any patient requesting the services of a named GP receives a call from that GP. Unfortunately this is not always possible as that GP may not be working on the day you call. Where this occurs an alternative GP will be offered.

We find that 70% of consultations conducted over the telephone do not require a patient to attend the surgery. This save our patients valuable time and ensures that they receive the help and advice on the same day that they make a request for assistance. If however you do need to be seen the GP making the call to you will arrange your appointment with them on the same day.

All requests are initially for telephone appointments.

Extended Access Appointments

As part of our commitment to provide an increasing range of GP Primary Care services for our patients and local community, irrespective of where you are registered we provide GP Extended Access appointments Monday to Friday from 18:30 to 20:00 and Saturday mornings from 09:00 to 12 noon.. The service has been commissioned by NHS South Lincolnshire Clinical Commissioning Group and is being provided by our Practice in conjunction with Allied Health South Lincolnshire (our GP Federation).

Appointments are pre-bookable in the usual way. If you are not registered with our practice but live within the community we are still happy to see you. Ask your GP Reception for an appointment within our service. You will be asked to consent to us viewing your medical record so that we can provide the best possible care and can only see you if this consent is given. Once given and you attend your appointment, our clinician will be able to view your personal GP record and any updates following your consultation will be automatically received by your Practice at the end of the consultation.

On Sundays and Bank Holidays appointments are currently available from 9am to 12 noon within our local area, via The Munro Practice in Spalding.

The Galletly Practice is providing this service in conjunction with other Practices across the area who are aware of our service and should offer you appointments if appropriate and available. We are working closely together so that you can take advantage of high quality care within your immediate locality negating the need to travel outside of Bourne or the immediate area.

To arrange an appointment or for more information on when and where appointments are available please speak to you usual Practice. This service also links into our same day appointment system for all our registered patients. Simply call us on the day you require assistance and your needs will be addressed that day by our GPs with no-one advising we have no remaining appointment and suggesting that you should call back another day. If not registered with us and this system would better suit your needs rather than waiting long periods to see your GP, our lists our open and we would be happy to register you as one of our patients.

For our Registered patients we now also offer Extended Hours appointments on Tuesday and Thursday evening between 18:30 and 20:00 and Saturdays from 09:00 to 12noon. These appointments are offered in the usual way via our Practice Appointment Line.

Other Appointments

If you require assistance regarding an appointment with one of our Health Care Support Workers, Phlebotomists or certain qualified nurse treatment room appointments, our receptionists will make your appointment direct with you over the telephone.

  • Mornings: 8.30am to 12.00pm
  • Afternoons: 2.00pm to 6.00pm

Tuesday and Thursday evening appointments along with Saturday morning appointments are for pre-booked appointments only and aimed at those patients who are unable to attend during normal hours due to work commitments etc. The practice does not operate any other services during this time and patients requiring emergency treatment should contact the Out of Hours Service (see advice on emergencies outside normal working hours).
