How To: Join The Practice

New Patient Registration

We always welcome new patients to our practice. All patients have a named doctor who has overall responsibility for your individual care and support. Patients are welcome to consult with any GP.

Following your registration as a patient with our practice we will allocate you a named accountable GP. This doctor will be responsible for your overall care at the practice and take lead responsibility for the coordination of all services at the practice providing your care. We refer to this as your “Usual” GP. If you have a preference for a particular doctor to be your usual GP, please discuss this with our reception staff and we will make all reasonable efforts to accommodate your request. In the case of nursing homes all the patients in a particular home will be allocated to the same named accountable GP because it is easier for nursing home staff to liaise with just one GP at the practice.

Our practice covers an area which has a radius of approximately 5.5 miles from the centre of Bourne.

How to Register

We welcome new patients to Bourne Galletly Practice. If you wish to join the practice you can do so by visiting the surgery during normal working hours or by downloading the registration form and visiting the surgery at a later date.

  • If you have your NHS number, please include this on the registration form. It does not matter if you do not have it but it may help us to trace your medical records more quickly.

You will be asked to complete a medical history form and then given an appointment to see your new doctor. You will also be given a booklet containing all the details about the surgery and the services we provide. This will complete the registration process.

Children 16 and under do not need to complete the registration form. An adult member of the family can register them by providing the required information and signing the registration application on their behalf.

You will be allocated a specific GP who has vacancies on their list. As a patient, you have the right to express preference as to which GP you wish to see. However, if you require an appointment and your preferred GP is not available, you will be offered an appointment with another doctor. If you still wish to see your preferred GP you may have to wait longer.

New Patient Health Check

All new patients aged over 16 are invited to make an appointment for a routine health check with a nurse or healthcare assistant. If you are taking regular medication or have any significant medical conditions we advise you to book an appointment with your new doctor.

Pre-Registration

To register at Bourne Galletly Practice, please complete the Registration Form below, along with the Medical History Form, Ethnicity Form and Screening Tools for Alcohol Form if applicable.

Please hand in the completed form(s) to the reception team.  Depending on the information you provided in the Medical History Form, you will be advised whether to make an appointment with a PHCA/Nurse.

Registration Forms

You can also visit the surgery and the above forms will be provided.

You will be asked you for personal details such as your name, address, date of birth, telephone number, previous address, previous doctor and your e-mail address if you have one. This information helps us to quickly trace your medical records request and them from your doctor.

Once you have completed and signed the form you will be automatically placed on the practice list.

On the back of your registration form, you are given an option to go on a central register as a donor.

If you need to see a doctor you are welcome to make an appointment at the time you register.

Your registration details will be stored electronically, which will speed up the appropriate administration processes. When you require an appointment in the future, this will enable the receptionist to action your request quickly and accurately.
