We offer a daily nurse led minor illness service. If, after discussion with you, the doctor feels that your problem is suitable he or she can book you in with the specially trained minor illness nurse.  The nurse will then take the most appropriate action. This may be advice, treatment or she may arrange an appropriate follow up appointment with another member of the primary healthcare team.

Our treatment room is staffed by nurses and nursing assistants. They provide a comprehensive range of nursing services including health screening. The treatment room is open from 0900 to 1200 and 1300 to 1745 hrs daily.

Some of our nurses are trained in chronic disease management. These nurses can review and manage diabetes, asthma, chronic obstructive pulmonary disease (COPD), Chronic heart disease, Hypertension and chronic kidney disease (CKD).  Appointments for our Chronic Disease Clinic nurses can be booked by calling us and speaking with the doctor.

The doctors will refer you for this service where appropriate. Due to the huge clinical demand for this service, self referrals cannot be accepted. A fully qualified physiotherapist provides a wide range of services at the Bourne Health Centre at St Gilberts Road, Bourne just a few hundred yards from the Surgery but other Physiotherapy services are available in Market Deeping, Corby Glen and Stamford.

41697799 s guzhvaMed 3 (2010) Fit Notes - formerly known as sick notes can only be issued AFTER a consultation with a doctor or nurse. 

Most fit notes are issued for the amount of time that your doctor thinks you will need to be off from work.  If you remain too ill to work, you may require a further fit note.  In some cases a phased return to work or altered work environment may be appropriate.  You will usually be expected to contact your doctor and book a telephone or face to face (if appropriate) appointment to discuss your situation.

If the doctor has already agreed that an extension would be appropriate then we will need the following MINIMUM information:

  • Your full name and address
  • The date you want your fit note to start from
  • Your medical condition, which is on your current fit note
  • The duration for which you need it e.g.: one week

We will endeavour to process fit note requests within two working days although this is not always possible.

There is no longer such a thing as a "signing off note", you can return to work when your current fit note expires
Time off for Hospital Admissions and surgery: General Practitioners are no longer responsible for certifying sickness where patients are admitted to hospital.  Hospital Clinicians are now required to issue you with a fit note for the time that you were in hospital and for the amount of time they anticipate that you will remain too ill to work after discharge. Please ensure that you ask the hospital staff for a fit note if you require one.

To access this download a  pdf Travel Vaccination Form from Patients Documents section (100 KB)of this web site or call in at Reception who can give you one. Use the form to set out your travel itinerary. Use a separate form for each person travelling as their requirements may be different.

Please hand the form in at Reception or post to us at "Bourne Galletly Practice, 40 North Road, Bourne, Lincolnshire, PE10 9BT".
Our Trained nurses will then work out what vaccinations are required for each person.  Our receptionist will then contact you with the cost of the vaccinations and once payment has been received you will be booked in for the vaccinations.

Weekdays  2pm - 4pm

  • If your doctor has sent you for special tests, you can telephone us on the Enquiries number between 2pm and 4pm for the results. Please allow several days before ringing in; if there is any urgency we will contact you. Do not make a further appointment until all the results have been received, unless you have been otherwise advised by your doctor.
  • When you have your tests, you will be given a personal identity number (PIN). This is an exclusive number relating to your medical records so please do not disclose it to anyone. When you telephone the surgery for your results, you will be asked for your PIN number before the information can be given to you.

All women are encouraged to have regular smear tests to detect pre-cancerous changes at the neck of the womb. You will be invited to attend routinely, but if you would like to make your own appointment, please ask the receptionist.

What is the Smear Test?

It is not a 'cancer test'. It is a test to pick up very early changes in the cervix which could become cancer in the future. However, 90% of smears are normal.

Results - What They Mean

  • Negative: Your smear test is normal which means you are at very low risk. However, should you experience abnormal bleeding at any time you should see your doctor without delay.
  • Inadequate/Unsatisfactory: Between 5-10% of smears are unsuitable for testing and therefore have to be repeated.
  • Abnormal Smears: Only 5-7% of smears are abnormal. This means that there are changes which, if left untreated, may become cancer in the future.

However, most of these changes return to normal without any treatment being required. For this reason you are usually asked to return for a repeat smear test in 3-6 months. If treatment is needed, it is almost 100% effective involving a minor procedure in the local Outpatient Department.

Results

You will receive your results by post to your home address in approximately 6-8 weeks following your smear test.

Minor surgical procedures are performed on the premises by some of the doctors. These are usually done under local anaesthetic. and include joint injections, the removal of minor moles, skin tags and small cysts. We can only remove clinically significant lesions (lesions causing a problem) and not merely cosmetic ones.

Your doctor will inform you if he or she feels that you have a condition which is suitable for such surgery. 

Please book a routine appointment with your usual doctor who will be able to advise you regarding the most appropriate procedure for your problem.

The treatment room nurses and health care assistants run a triage system. Basically, this means that patients are initially passed to the nurse, who assesses the complaint and decides the most appropriate response to each request. This can be accessed on a daily basis, in cases where a patient may have an acute condition or emergency. If the nurse cannot deal with the problem, then she will seek the advice of the Duty Doctor. During the afternoon from 2.00 pm to 5.00 pm we hold a minor illness clinic. This is bookable on the day by telephoning. Problems suitable for this clinic include:

  • General medical advice, i.e. colds, flu, headaches, sore throat (usually dealt with by telephone and there is usually no need to be seen for these)
  • Rashes, stings and insect bites
  • Urinary symptoms
  • Earache
  • Conjunctivitis
  • Emergency contraception
  • Some wounds - cuts, abrasions, animal bites
  • Minor injuries e.g. head injuries, sprains
  • Impetigo


In addition to this emergency service, the nurses also perform all routine nursing services. e.g.

  • ECG's
  • Blood Samples
  • Blood Pressure Checks
  • All Immunisations
  • Contraceptive Advice and Monitoring
  • Teenage Health
  • Emergency Contraception
  • Travel Advice and vaccinations.

Requests must be made before 1100 hrs as they are allocated and the visits made between 1230 and 1400 hrs.  We always prefer to see you in the surgery where we have better facilities diagnostic aids and lighting to assess you. If, however, you are too ill to attend, then a request for a home visit can be made by telephoning the number above. Be ready to provide some details of your problem and your telephone number.  A doctor will then telephone you back in order to assess your request.

EMERGENCY VISITS, for life threatening situations, are undertaken at any time, but it is usually more approproiate to dial 999 in these situations and you may be asked to do this by our staff or the duty doctor.
