Repeat Prescription Notes

This service is no longer available as we have migrated our service across to the newer On line Booking and Prescriptions service.

 

ORDERING A REPEAT PRESCRIPTION

When you order a repeat prescription order online, we need to know the name(s) or description of the drugs you require, their quantities and their strength. You can find this information by checking the paper form you received with your original prescription form as shown in the example below. Equally important is the name of the original prescribing Doctor. Note:- this is the name of the doctor who prescribed the drugs who may or may not be your regular doctor. Other form requirements such as name, date of birth, etc are self explanatory.

If you do not have an original form to use for your repeat prescription then please contact the surgery for advice (click on the form below to display a sample copy).

FILLING IN THE FORM

Items marked with an This information is necessary and must be supplied are mandatory, but the more information you supply the easier it will be for us to contact you should there be a problem or delay with the order.

Fields coloured red are either incomplete or incorrect. Usually this happens when you try to save your prescription details or try to submit the form. Please correct them and re-submit.

Items marked with a symbol have additional help information. To view the extra information just move your mouse pointer over the symbol

To clear all fields then click on the button near the bottom of the form called "Clear form"

SAVING YOUR PRESCRIPTION DETAILS

In order to make repeat prescriptions easier to make when you revisit this form,  data can be stored locally on your PC as a cookie by clicking on the "Save Prescription data" button, once you have filled in the form the first time. To understand what a cookie is then click here.

If you do not wish to store this data for a future repeat order, then you only need to submit the form once you have typed in all the necessary details. Although saving the data is considered safe, we have specifically made this optional. If you wish to fill in the form in full each time then you are free to do so.

FAMILY MEMBERS DETAILS

In order to allow more than one family member to save prescription data or populate the form with previously saved data, you will need to specify a user name ( e.g. johnsmith ) to identify individual prescription orders. 

On subsequent visits the form can be filled in automatically by typing in a previously used username in the relevant box.  If  a repeat prescription has been changed since it was last saved, then remember  to "Save Prescription data"  again once you have amended your latest requests on the form.

CLEARING FORM OR PART OF FORM

You can either clear the whole form or just clear individual prescriptions you no longer require. Typically you would do this after you have retrieved your last saved details. Beside each prescription entry there is a "Clear" button. this will only clear the prescription details on that line.
